Hospital check up form
Hospital intake form where children can mark where the animal is hurt, what they recommend as treatment and add their name! My students in my class are three years old and we are learning about veterinarians and doctors. To accompany this, I set up a veterinarian center in the classroom. Offered stethoscopes, bandaids, gauze, ace bandages, xrays, fake medicine, doctor's clothes and different stuffed animals. The children were able to pick a patient, diagnose them and treat them in any way they saw fit. The children took turns using a clipboard and check up form to help their patients and then told their friends all about it after.
